 For faster service, you are encouraged to complete the "health questionaire"
 (Page 1) of the IHSAA Physical Form BEFORE YOU ARRIVE ...

All players MUST provide ZYFL a Physical Exam Form completed by a physician after November 15, 2007.  If you want to use your child's physical exam for school sports during the 2008-2009 academic year, it should be performed ON/AFTER MAY 1, 2008 to be accepted by Zionsville Community Schools.
