Risk Management Info.

Information regarding the Risk Management program and the application form can be found in this category.

Risk Management Form and Information

New coaches or existing coaches - if you do not have a risk management card or yours has expired please fill out this form and forward to the Michigan State Youth Soccer Association (Address at bottom of form)

Please make sure to include a copy of your drivers license as the form can not be submitted if you do not include a copy.

You may also request a Risk Management card through the MSYSA. Click on "Links" on the left of this page then find the Michigan State Youth Soccer link and click on that. Click on "Coaches" on their home page.

Risk Management cards have a $10.00 fee.

Registration Material - Any child that has not played with the League must have these forms filled out and turned in along with a copy of their birth certificate AND a small picture for use with their player pass card at registration time.
