- All files are readable using Adobe Reader. Download and fill out the forms. Then bring the completed forms to one of the sign-up days.
Registration forms
- Fill out a separate Player Registration Form and a Medical Release Form for each player.
- Be sure to select a uniform size. Sample uniforms will be on display at the Sign-up Location.
- NOTE: There is $30 late fee for any registration form received AFTER January 31st.
- Washington Township Baseball, Inc supports the Little League Child Protection Program. A completed Volunteer Form (plus a copy of a photo ID) is required for all candidates for managing or coaching any team in any league.
