**TEAM OR PLAYER REGISTRATION**
PLAYER ID CARD REQUEST:
Everyone who plays in the league must have a player ID card to check-in with. To obtain a card, follow the instructions below:
1. Download/complete waiver
a. Fax to 1-866-431-4864 OR
b. Scan and email to wsosgv@hotmail.com OR
c. Mail to ASL attn: Sherisse, PO Box 61063, Pasadena, CA 91116
2. Send photo of yourself (returning players can skip this step unless they want to update their photo)
a. Email as jpeg or bitmap to wsosgv@hotmail.com OR
b. Mail photo in and we'll take a picture of it (color only, min 3"x3"). We will crop it for you.
3. Pay online or by mail for ID card as follows:
a. $15 for player card
b. $20 if adding after the third week of games
c. $5 if purchasing a duplicate ID card (suggested is you play in more than one league)
Once EVERYTHING is received, the card will be processed and mailed to THE CAPTAIN of the team you're playing on, unless otherwise specified.
INDIVIDUAL PLAYER'S LOOKING FOR A TEAM TO PLAY ON:
Please fill out the questioner at the bottom of this page so we can better place you in the league according to your skill level. Since this is not checked daily, you should send a follow-up email to wsosgv@hotmail.com to let us know you filled out the form.
The majority of the leagues are 'team registration' only. It may take a while to get placed onto a team since it depends on whether a team needs a player and/or if a league is in the middle of the season. Every so often, we will form a team, but only if we have room for a team, AND enough players are interested so we can form it.
The best time to contact us regarding player availability is approximately one week before the league begins.
TEAM REGISTRATION:
Due to limited space, we require all interested teams to submit a $150 deposit to be added to the waiting list, that way we know which team's are serious about adding. How it is determined whether a new team can be added:
1. Teams in the current session get priority registration over teams on the waiting list. If a current team does not pay on their priority registration date, then teams on the waiting list will replace those teams.
2. To get onto the waiting list, the team manager needs to submit a $150 deposit. Once received, they will be placed on the waiting list in order of deposit received. This eliminates any problems of who gets added first.

Note which league it is for and send a follow-up email to wsosgv@hotmail.com. The captain can also submit their information in the 'new player information' form below.
Please note: The deposit will be deducted from the total fees due. If the team does get in and they choose not to play, then the deposit is forfeited.
