HOW TO REGISTER FOR THE WSOSGV/ASL PLAYER ID CARD
Everyone who plays in the league must have a player ID card to check-in with. To obtain a card, follow the instructions below:
1. Download/complete RELEASE OF LIABILITY form
a. fax to 1-866-431-4864 OR
b. scan and email as an attachment OR
c. snail-mail to ASL attn: Sherisse, PO Box 61063, Pasadena, CA 91116.
2. Send photo of yourself (RETURNING players can skip this step unless you want to update your photo)
a. Email as jpeg or bitmap to wsosgv@hotmail.com OR
b. Mail a color photo of yourself (3"x3" min). Don't worry about cropping it; we can do it. We do not accept fax pictures.
3. Pay online or by mail for ID card as follows:
a. $15 for ID card ADD ON:
i. $5 if you would like a duplicate ID card
ii. $5 if adding after the third week of games
iii. $5 if you want the ID rushed (less than 5 days before game time)
Once EVERYTHING is received, the card will be processed and THE CAPTAIN will check back to get your card.

Deadlines are listed below for each league. Once everything's turned in, we require a minimum of five days to get the card processed and mailed out. So for example, if you want to play in the next Tuesday game, you will need to submit all three items by Thursday (the week before), to receive your player ID card in time.

Administrative work is NOT done at the field, period! You can email anytime, to check on the status of your card or need help registering.
Since the PO Box is only checked every few days, it could take longer than 5 days to process your player ID card. The quickest way to obtain a player ID card is to fax in the waiver, email your picture, and pay the fee online.
REPLACEMENT OR ADDITIONAL PLAYER ID CARDS.
If you play in more than one league, we recommend you get an additional player ID card. To get another card, submit the $5 payment online or by check, within the date listed above.
