Both forms listed below must be completed for Fall 2009 registration. Please include payment of $60.00 for each registrant along with a small COLOR photo and a copy of the Birth Certificate for each player unless you supplied a copy of the birth certificate for the Fall 2008 season than you do not have to send a copy of the Birth Certificate. You do need to send a new photo for the player pass.
