All forms are in .pdf file format so you may easily view and print them.

The Player Handbook, Player Questionaire, and ConductResidency and Medical Buff card all must be completed prior to tryouts.

Print the Medical Buff Card and have your physician complete it, then bring it with you to give to your coach. If you've already completed a Medical Buff Card for another sport this school year, let the coach know, as you will not need another.

Note that the Master Schedule in the Handbook has been updated. Please replace it with the 2009 Master Calendar below.
