Late Registration:

What you need to turn in ASAP: (per child)

•  Registration form: (Go to Handouts/Forms to download)
•   Medical release form: (Go to Handouts/Forms to download)
•   3 checks: 
          1) One check dated with current date for registration fee*. 
          2) One check for $50 dated 6/1/12 for uniform deposit (softball only)
          3) One check for $200 dated 6/1/12 for snack stand/field duty deposit
             * You may combine multiple children’s registration fees in one check.
     Deposit checks must be two individual checks for each child.
•   Copy of Live Birth Certificate –
(Abstract is not acceptable)
•   Utility Bill or Drivers License for proof of residency within boundary

Softball Fees:
$165 per player  
$140  per additional player
Tee Ball Fees:
$120 per child
