When printing the Registration Waiver of Liability/Photo etal Form, please print the second page on the back side of page one to reduce paperwork.

Football and Cheerleading Preseason Camp Registration Form. ALL participants must complete and provide this form to participate. A physical, dated 2008, must be provided or physician's statement may be signed. Players and cheerleaders that were active on roster at the end of the season may utilize the physical provided.
