| All Participants must completely fill out and have their Parent or Legal Guardian sign both Registration and Liability Forms. They must have a copy of their Birth Certificated and payment of $150.00 to complete the Registration Process. |

- All Coaches must read, and sign the Coaches Code Of Ethics before the can be approved to Coach in our WASUP League. Applying to Coach does not guarantee you to Coach in the 2010 Season. All Coaches must be approved to Coach and or get a team.
