Uniform Order Request
Coaches and Team Managers:
The process for ordering uniforms is outlined below:
Note there are are three different order forms to choose from - Girls, Boys and Academy (boys & girls)
- Each team's uniform order needs to be on ONE form and emailed to me at amijdavis@msn.com
- if you need to, please attach a separate spreadsheet to list numbers for jerseys or other items
- do not create your own order form, please use the one provided
- you can add the extra costs (such as embroidery or latex #) to the side of the order form on that same line
- the item that you want the # or name on - prices for the extras are at the bottom of the sheet
**jersey numbers are included in the price of the jersey**
- Make sure you put your team name at the top of the order form, the age and the coaches name
Example: WSC Thunder U14B Dan Thompson
- Orders take approximately 3 weeks and may take longer during peak times (right before a season begins) so plan accordingly
- Orders will be placed on the 1st and the 15th of every month (if you send your order form to me on the 2nd it will not be ordered until the 15th - again, please plan accordingly)
- Orders do not need to be paid for until they are picked up. I will let you know when the order is in and you can make arrangements to pick it up. Bring ONE check for the entire order payable to Wasatch Soccer Club
- There is a sizing chart on the website as well, please use it if you are unsure about what sizes to order.
- Orders cannot be returned because the wrong size was ordered.
