Sports Clearance
All players are required to get medical examinations (physicals), complete and return all sports clearance forms, and pay a transportation fee prior to the start of the baseball season. To do so, please obtain a "Sports Clearance" packet from the VHS Attendance Office (located in the admin building). This packet contains:
- a medical examination (physical) form with verification of medical insurance
- code of conduct form
- address verification form
- coach's emergency card (yellow)
- instructions for paying a transportation fee
- instructions for completing & filing all paperwork, which includes verifying all obligations (student store/library) and all detentions served
Players need to return all completed forms to the Athletic Secretary (AS). After the AS verifies everything is done, she will sign then give you a green "VHS Athletics Dept CLEARANCE FORM" which is to be given to Coach Togneri along with the yellow coach's emergency card. >>> Clearance must be completed before first official practice: February 15, 2010 <<<
