***ONLY NEW PLAYERS IN 8TH GRADE IN SEPT 2013 WILL NEED TO PAY A $200 EQUIPMENT DEPOSIT IN CASH OR MONEY ORDER WHICH WILL BE RETURNED AT END OF SEASON PROVIDING EQUIPMENT IS RETURNED.  THIS DEPOSIT WILL BE DUE BY JUNE EQUIPMENT HANDOUT****

1) 2013 POP WARNER PLAYER CONTRACT:

Middle Initial Required,  Parent/Guardian CANNOT BE  the same as emergency contact person, EMAIL mandatory as well as best phone contact number- CELL PHONES preferred

Both Parent AND Player Signature required on Side 2 

2) 2013 Tackle Payment Form-

completely filled out with correct weight

and all information  (WILL BE RETURNED FOR ANY MISSING INFO)

3) CODE OF CONDUCT- signature page ONLY  

4) PHYSICAL FORM-

must be dated by physician Jan 1, 2013 or later - actual Physical Date can be prior to that date,

make sure ALL information is completed on both sides- Parent side 1/Physician side 2

DO NOT SEND SIDE ONE WITHOUT SIDE TWO
