Parents must complete a registration form and parent code of conduct for each player. New players to the league must also submit proof of age (birth certificate),proof of residency (3 documents required - driver's license, utility bills, bank statement, etc.) and a parent code of conduct form.   

Returning players may mail completed forms with the registration fee to Plymouth Little League, PO Box 191, Terryville, CT 06786.
