Registration Process 

1.  Prior to August 15th, go on-line to register with USA Hockey at www.usahockeyregistration.com .  The USA Hockey fees are $40 paid with a credit card or debit card.  

2.  Click on "Ice Players & Coaches" and follow the directions.  If under 18 years old, a parent or guardian must complete the process.  All players and coaches are required to register online with USA Hockey.
 
3.  PRINT your receipt with confirmation number and bar code.  This is very, very important.

4.  If unable to attend registration on August 15th, contact your team registrar (listed below) to submit the following documents to complete your registration prior to August 15th.

a.  Original USA Hockey Confirmation Receipt (keep a copy for your records).
b.  Copy of your birth certificate if this is your first year to register with Tulsa High School Hockey.
c.  Copy of your student ID or school schedule.
d.  Completed, signed USA Hockey Consent to Treat Form.
e.  Completed, signed USA Hockey Code of Conduct.
f.  Completed Player Information Sheet.
g.  Check payable to your High School team in the amount of $150.00.  Five additional post-dated checks in the amount of $125 each, dated 10/01/09, 11/01/09, 12/01/09, 01/01/10, 02/01/10.  There will be a final amount due 03/01/10 that will be determined once we have final registration numbers.

No player will be allowed on the ice until ALL registration papers and payments are received.
