Registration Documents

Please print out the Registration, Medical Waiver and Code of Conduct, fill them out completely, sign them, and bring everything to one of the in-person sign up dates. You must bring a copy of the child’s birth certificate and any 3 different proofs of residency

from the following list:

Drivers License;
Banking/Credit Card Statement;
Voter Registration;
Insurance Documents;
School Records;
Internet/Cable/Satellite Records;
Vehicle Registration;

(Do NOT bring Electric, Gas, Water/Sewer/Garbage, telephone and cell phone bills as your 3 proofs. Utility bills are considered 1 Proof.)

Volunteer Application

Anyone working with youth - managers, coaches,team moms, etc. - must pass a background check. Please fill out the volunteer form completely, including social security number, sign and date it.
