South Whidbey Little League: REGISTRATION

Below is a list of forms you need in order to register this season. All you need to do is click on the form, print it and fill it out.

Here is what we will need from you:

*Completion of the following forms:
    Player Registration Form
    Medical Release Form
    Parent Code of Conduct Form
    Concussion Acknowledgment Form   
*Copy of player's birth certificate
*Payment (see Fees below)

You may bring these forms with you to clinics or try-outs. You can also mail them to:

Clinics: $20 (will be applicable to reg fee)
Season fee for All ages: $100
Family Cap: $250
