SWAPW is happy to announce that On-line registration is available once again. SW will continue to accept mail-in registrations with the following registration documents, to the address listed below. Make checks or money orders payable to SWAPW.

  • 2008 registration form
  • 2008 physical form
  • 2007/2008 Year End Report card-report card must contain all grading periods.
  • Parents Code of Conduct
  • Photo copy of original/state issued birth certificate
Bulldawg parents it's no suprise how fast roster spots are filling up across the board. SWAPW strongly encourages you to lock down a spot for your child now, as it may be too late to do so in the coming weeks.

 

Remember roster spots will be limited per team as SW will only field 8 teams this year.

Registration:

 

Tackle registration  - $150.00 - parents will need to provide their own tackle equipment, such as helmet (black only), shoulder pads and game pants.  SW will provide game jersey, required pop warner patch and trophy as part of the registration fee.

 

Cheer - $50.00 -  required pop warner patch and trophy will be provided. The registration price does not include the uniform. 

 

Cheer parents will be responsible for the cost of uniform and accessories.  SWAPW's Cheer Coordinator will send out details regarding the cost of the uniform and accessories.  Approximate cost for uniform is $250.

 

REFUNDS:

 

Refunds will only be given in the event your application for membership is not accepted or in lieu of a dropped participant and requesting a refund prior to August 1st.

 

Refund amounts will be based on time of dropped participant.
Prior to July 1st-100% registration fee will be given.
July 2nd-July 14th 75% registration fee
July 15th- August 1st 50% of registration fee.

**SWAPW will not issue an equipment refund (helmet & shoulder pads).**

**REGISTRATION REFUNDS WILL NOT BE ISSUED AFTER AUGUST 1st.**

 

Update on Tackle Equipment Sale:

 

SW will conduct a tackle equipment sale time and location TBD (time and location to be determined) items listed below will be sold on a first come, first served basis. See pricing below.

  • Helmet-$40.00
  • Shoulder Pads-$20.00
  • Practice Pants-$10.00

Checks must be made out to SWAPW

 

Tiny Mite Tackle Equipment:

 

Tiny Mite participants will be required to purchase the following equipment, (black helmet and shoulder pads) since we may not have that size of equipment for sale. If that is the case a vendor will be recommended to our members at a cost savings. 

 

The following registration documentation is required:

  •  Registration Participant Form
  •  Original and copy of Birth Certificate (must have state seal) 
  •  Physical - dated after January 1, 2008 (Must use the required physical form located in
     the Forms section)
  •  End of year report card for 2007-2008
