| Attention Bulldogs...2008 Sport Physicals Exams will again be offered in Swanton for student athletes in grades 6 through 12. The cost is $10.00 (CASH ONLY) per person, and no appointment is necessary. Participants will be seen on a first come basis. Students MUST BRING a completed “Ohio High School Athletic Association Participation Physical Evaluation” form signed by a parent or legal guardian PRIOR TO the exam. This form can be found on the internet at: http://www.ohsaa.org/medicine/PPE.pdf or picked-up at your School Office or the Swanton Board of Education Mon.-Fri. 8:00am-4:00pm. Unless your physical condition changes, this physical will be good through the summer of 2009 (should cover all upcoming sports). Local and area medical personnel and community volunteers are donating their time to help our student athletes. All proceeds will be used to help fund Swanton athletes.
