Just an FYI, we WILL NOT be accepting the registration forms without the $50 registration fee.
MEDICAL RELEASE FORMS AND PLAYER CONTRACT FORMS WILL BE UPLOADED SHORTLY......
The deadline to get all registrations in is August 1st. Make sure you contact Sophia Snyder (Cheer director) at 408-644-6924 and/or Don Horrigan (Football director) at 408-306-1482 for further information. Thank you.
*Please note: There's a non-refundable registration fee of $50 due at the time of sign-ups plus $225 participation fee due by August 1st. There will also be a $75 dollar volunteer fee added to the participation fee. If 2 or more children sign up, the volunteer fee would be less -$25 per additional participant. Once the 6 hours of volunteer time (10 hours for 2 or more participants) has been completed, you will be refunded the volunteer fee paid at the time of registration.

Registration Update
Mandatory - Due on or before August 1st.
* Medical Release (must use 2009 Pop Warner Medical Form, exam date anytime in 2009).
* Report Card (two copies of the entire 2008-2009 school year).
* All outstanding fees (credit/debit cards are accepted for payment).
