  • Accident Claim Forms - If you have any type of injury, no matter how minor, always refer to the Accident Form Instructions AND contact the league's Safety Officer right away.
  • Player Registration forms - Speed up your registration process - download, complete, and print the Player Registration and Medical Release forms. Please make two copies of each form so that we can complete the registration process. Don't forget to bring your player's birth certificate and three verifications proofing league residency.
  • Manager Survey - A survery designed for parents to give feedback to the Sutter Area Little League board regarding their child's experience in the Little League program.
