Concussion Awareness Requirements and Form

With the blessing of the NFL, the state of Arizona passed concussion legislation that was signed into law April 2011. As a result, all athletes and their parents must be educated in the signs, symptoms and what to do if you have a concussion. Coaches must also be trained regarding the same. In order to use school facilities for our programs and our tournaments we must comply with this new law. As a result the Arizona Region requires that each parent and player read their respective CDC Fact Sheet on Concussions and sign an Acknowledgement Form indiciating they understand the signs, symptoms and what to do if they think they have a concussion. The Acknowledgement form is to be turned in to the club with registration materials. No player will be put on a roster until this form has been received each season.

SafeSport Information and Parent/Participant Form

The US Olympic Committee, USA Volleyball and the Arizona Region are on board with the

SafeSport program for the 2015 season and beyond. It is a USOC mandate that all participants and

their parents are trained in SafeSport and policies are established to reduce abuse in sport. More

will be rolled out as the program takes hold. For 2015 the following must be met for the Arizona

Region to be in compliance:

By Dec 31, 2014:

1. Everyone in leadership in the Region must take the SafeSport Training.

2. The Region must establish a SafeSport Contact.

3. Each club must establish a SafeSport Contact in their club.

4. The Region and the clubs are to establish Travel policies and Social Media/Communication

policies within their organization that comply with the SafeSport Program. If the Region

or club does not establish the policies the default is the SafeSport policy provided by

USAV in the SafeSport Handbook.

The Arizona Region has also created a SafeSport Program and Document to start the education

process for the parents. The document is the definitions of the 6 areas of SafeSport. The program

document is to be read by the parent and discussed with their athlete. The parent then signs the

form (page 3 of the document) and the form is required to be filed with the Region (by the club) to

complete the membership process.
