Section 1--PURPOSE OF FORM AND INSTRUCTIONS FOR SUCCESSFUL REGISTRATION

This form signifies your team's commitment to play Super II in the Fall 2002 season.

INSTRUCTIONS:

Enrollment in Western District Super II play is open. However, local association rules still apply. Therefore,

a team must first satisfy local association rules before registering for Western District competition.

To be scheduled for Super II play, the following conditions must be met:
