Qualification Tournament June 28 - 30, 2002 Entry Form

Section 1--PURPOSE OF FORM AND INSTRUCTIONS FOR SUCCESSFUL REGISTRATION

This form enters your team in the Division I Qualification Tournament and, if you qualify, signifies

your team's commitment to play Division I in the Fall 2002 season.

INSTRUCTIONS:

To be scheduled for Division I play, the following conditions must be met:

On or before June 20, 2002 the appropriate tournament organizer must receive:

$400.00 check made payable to CAYSA (U12 - U14) or AAYSA (U15 - U19)

team roster--must be typed and include player ID and player's first and last names

Team must qualify in tournament

(3)

Upon completion of tournament, team must submit $25.00 check payable to Western District

(4)

On or before August 1, 2002 the appropriate Western District coordinator must receive a

signed fall 2002 club or association roster. Failure to return at least 90% of the players from the

qualification tournament roster will render the team ineligible for Fall 2002 Division I competition.
