Beginning in the Fall 2008 season, St. Elizabeth CYO Soccer Club implemented an online registration system. Online registration provides convenience for our members while streamlining club administration.

Registering online is as easy as 1-2-3, you will be asked to:
1) set up your account by supplying
    a) parent info
    b) info about each of your children
2) Register each child
    a) choose the appropriate program, division
3) Payments can be conveniently made via credit card or via check

FALL 2008 REGISTRATION LETTER

TOPSoccer (The Outreach Program for soccer) is a soccer program designed to meet the needs of children with physical and/or mental disabilities. This program consists of a one hour weekly clinic involving fun drills and a small sided scrimmage. Teams are created based on ability not age. This program is for any child, aged 4-18, who cannot participate on a regular team. Clinic is held on Saturday mornings at 10:00 at Maplewood School. The cost for this program is $40.

    Please fill out the top half of the form & hand it to your referee/s at the beginning of each game. The form is a twin & is MS Word 97 document format.

USYSA Medical Release Form - S04

    This form is in Adobe "pdf" format. To be filled out by the coach when an injury occurs. The team's appropriate Coordinator should be contacted for direction.

Advance Notice of Injury

    This form should be filled out by a travel coach after each game and sent into LIJSL. This is the best way for the referees to be critiqued & rated. Please do your part!

LIJSL Referee Evaluation Form

    This form is in Adobe "pdf" format. You'll need this form if you are travelling out of state for a tournament. It needs to be handed in to LIJSL a minimum of 30 days prior to the tournament for approval.

Permission to Travel

    This form is to be used when adding an existing travel player to your tournament roster. This form needs to be handed into LIJSL at least two weeks prior to the tournament!

    To be filled out for the addition of an intramural player to play in a tournament on a travel team. This form needs to behanded into LIJSL two weeks prior to the tournament!
