Coaches, Assistant Coaches, Team Parents - Disclosure Form/Background Check

image_name2

Every adult that is assigned to a team, whether you are the Head Coach or the Team Parent,  MUST fill out the on-line disclosure form so the league can run the  required  background check.   This must be done every season so we have your current information.

Click HERE to access the secure page, enter your information and then submit.  You'll get a confirmation page, which you can print and hold on to as your proof that you have complied until you get your badges (coaches and assistant coaches only).

All information must be filled out - including your social security number.  Your social security number is obscured in the data that we see - it is not accessible by the soccer league.

Any adult assigned to a team that does not fill out the form will not be able to participate in practices or games!

Soccer SLAM 2012 Results

U8 Divisional Championship: Winner:  Blue
Blue and Purple teams marched through their early games, defeating Orange, Red, Black, and Green in the process and faced each other at 11:30am, where Blue handed Purple their first defeat of the day.  Early in the secondary bracket, Red and Green sent Orange and Black home with Red ending Green's day in the 11:30am game.  Purple, after moving into the secondary bracket after their loss to Blue earlier, faced Red for the second time that day.  Purple triumphed over Red once again to move to the Final Game to face Blue.  Blue came out on top to win the U8 Divisional Championship.
U10 Divisional Championship Winner: Green
Orange moved into the late morning games, winning against the Blue and Black teams, relegating both to the secondary bracket.  Green forced Purple into the secondary bracket in their initial meeting and went on to upset the Orange teams momentum, forcing orange to the lesser bracket as well.  In the secondary bracket, Black and Blue overtook Purple and Red to face each other in a late morning game where Black pulled out a win.  Black then faced the recently defeated Orange team and pulled out another win.  It was Black versus Green in the Final Game where Green was able to defeat Black in a PK shootout to win the U10 Divisional Championship.
U12/14 Divisional Champoinship Winner: Green
Orange plowed through their early games, sending the Blue, Black, and Green teams to the secondary bracket. Red overcame Purple twice that day, once in each bracket, only to be bumped later by Green in their secondary bracket game. That win propelled Green into the Final Game to face the undefeated Orange team. Green triumphed in an extended PK shootout and won the U12/14 Divisional Championship.
The Shorewood Soccer League hopes all the players had a good time participating in this in-house tournament!  Thanks to the coaches and parents who extended their Saturday for this event - the children surely appreciated your support.
