Mandatory form for coaches & board members

This form required by Soccer Maine for all coaches, board members, volunteers that come into contact with kids, and team administrators.

Volunteer Disclosure Form

This form is adapted from the Soccer Maine code of ethics. All coaches have been bound by this code since the 2000 season, but SSC is asking coaches to sign to ensure they are fully aware of the code.

Expense Reimbursement Process
If you have an expense that has been previously approved for reimbursement
by an authorized SSC Board member, then please print out a copy of the
Expense Reimbursement Form below, follow the instructions to fill it out
complete, and mail with the receipt to:

A check will be mailed to you approximately two weeks after the treasurer
has received all information.
