South Valley Little League: Registration Forms/Important Info

Registration & Player Evaluations Dates Scheduled for the 2010 Season
At the Little League Office (1115-B Hwy 99N) -
          On the following Saturdays, from 10 .a.m. to 4 p.m.
January 23rd         January 30th            February 6th
See below for very important information about what you need to come prepared with when registering your child for Little League.

* "Player Evaluations" are the league's opportunity to help be sure your child is placed on the team best suited for their skills while keeping the divisions of play as equally balance in skills as possible.  Tee-Ball age youth do not participate in 'evaluations.' 
Basic drills used generally include:
 hitting, catching, throwing, infield, and outfield.
The site for evals has not yet been finalized.  Check back for updates or inquire when registering your ballplayer come January.  Players should arrive with a glove (if you don't have one, check out Goodwill, or visit with the league to see if there have been any donations.  Be sure your child is dressed in layers with respect to the season and time of year, in good 'running shoes' .
Additional information specific to "evaluations" may be obtained during registrations and/or by calling the league at 541 942 3841.
As of this publication date, evaluations are calendared as follows:
Saturday, February 20th - Girls Softball - 10 a.m to 2 p.m.
Saturday February 27th - Baseball
"Minors" - 10 a.m. to Noon
"Majors" (including returning Majors through league age 12, and all league age 12 baseball players)   -  Noon to 2 p.m.
"Juniors" (league ages 13-15) - 2 p.m. - 4 p.m.

Complete form and submit to the League President - with a photocopy of your government-issued photo I.D.

 All Board Members and anyone working with the children during the season must complete and submit the form and a photocopy of your government-issued photo i.d. (generally a drivers license). 

Little League requires we run a background and sex offender registry check on all who will be working with the kids - to provide as safe an environment as possible.

We are required to view the original government-issued birth certificate for your child to confirm their date of birth prior to their being assigned to a team.  This is strictly for viewing - and the document will be immediately returned to you at the time it is viewed.  Please bring this document with you when you register your child with Little League.

This document is NOT the "hospital record or hospital certificate" - but the actual government-issued document.

If your child is selected for an All Star team (post-season), our District Representative will need to view the document and obtain a photocopy for the tournament team affidavit packet.  Those packets are strictly and confidentially handled by the Manager of the All Star team and, post tournaments, it is shredded.

IMPORTANT REGISTRATION INFORMATION - including registration and medical release forms

Print off the SVLL Registration and Medical Release forms so you can complete them ahead of time for Registration!  Reminder:  For registration and before participation in any LL activity, we will need to view the child's original or certified copy of the GOVERNMENT-issued birth certificate as well as 3 proof of residence documents (suggested for ease:  1 drivers license, 1 utility bill, 1 motor vehicle registration or insurance document - and all must show the physical address of their legal residence.  Two or three of any one type of item constitutes "1" item).

 Little League bases its registration on the boundaries of the local League. The current boundaries are listed at the top of the "Site News" page.

If your child is a 'special needs' child (with physical and/or mental challenges that would make participation on a regular team a "challenge,"), please check out our page specific to "Adaptive Baseball/Challengers!  This division is set up for ages 5 through 21, or completion of their high school equivalency!


The "Regular Season" of Little League is an "every child plays program" - (Confidential "sponsorships" available on request).  There is a 'post-season' tournament program that is much more competitive and with more requirements for participation.

"LEAGUE AGE" - AGE REQUIREMENT --
In 2006, Little League and all other national Baseball and Softball programs moved to a 'universal' date for age determination of their players.   

TO PARTICIPATE IN Co-ed TEE-BALL & BASEBALL IN 2010 -
          birthdates MAY 1, 1991 through APRIL 30, 2005.

TO PARTICIPATE IN GIRLS’ SOFTBALL IN 2010 -   
          birthdates JANUARY 1, 1991 through DECEMBER 31, 2004.

TO PARTICIPATE IN THE CHALLENGER DIVISION® ("physically and/or mentally challenged youth") –
          birthdates May 1, 1989 through April 30, 2004.
"Challenger Division children may participate from Age 5 through graduation from their high school program - generally age 21.

For all regular season player registrations remember to bring with you:

* your Child - for uniform shirt sizing (or be prepared with sizing for 50/50 shirts)

* Per Little League and before a child can participate in 'evaluations,' or be assigned to any team, the League President and/or Player Agent will require viewing of your child's original Government-issued Birth Certificate (or a certified copy)

     Note: Passports, Hospital Certificates, Baptismal Certificates, photocopies or faxed copies of state-issued birth certificates cannot be accepted.
         * Military Hospital Certificates and Military I.D. Cards may be used in lieu of a "state-issued Birth Certificate."
        * Per the State of California, and due to 9/11, no California ABSTRACT of Birth is considered a legal document. (Contact the State of California or the County where your child was born to obtain a legal document - or click on the leader line for this article.)

IF YOU NEED TO ORDER A GOVERNMENT-ISSUED BIRTH CERTIFICATE, EITHER CONTACT THE COURTHOUSE OF THE COUNTY WHERE THE CHILD WAS BORN, OR GO TO WWW.VITALCHEK.COM. (PAYMENT CAN BE MADE USING A CREDIT CARD; SHIPPING IS VERY PROMPT.)

* and at least three different documents with your current address. (Documents referencing a PO Box must include a reference to a Street Address.) Authorized Proof of Residence documents may include a combination of three of any of the following, but not more than one document from each category. FOR EAZE, we are requesting it be kept simple - with the showing of:
      (1) a Current Driver's License
      (2) a Vehicle Registration
      (3) a Utility bill less than 6 months old or Insurance Document (with the address shown)
All three documents must show the same address as will be noted on the registration form, providing the league with proof the child resides within the legal boundaries of THEIR league..

Beginning with the 2006 season, players league age 12 this season may be eligible to "try-out" for placement on a Junior Division team. In Softball, there is no change to the ballfield.  In Baseball, this means moving up to the 90' basepath. Again, this option must be voted in for each season by the local board, and the ruling must be either "all 12 year olds" or "no 12 year olds" will be eligible to "try-out" for moving up to the next division.   Whether they "try-out" for placement on a Junior Division team or not, 12-year olds are always eligible for placement in the Player Pitch or Major teams which generally include 9-12 year olds - playing on 60' base paths.

The complete list of eligible "proof of residence" documents includes the following. ONLY ONE DOCUMENT FROM EACH (GROUP) is allowed. (2 drivers licenses = 1 proof of residence from group #1; a cell phone bill and a home phone bill = 1 proof of residence from group # 10).   ENVELOPES and Bank Statements and Checks do not qualify as a Proof of Residence Document.

(5) federal record;
(6) state record;
(7) local municipal record;
(8) support payment record;
(9) homeowner or tenant record;
(10) utility bill - i.e. gas, electric, water/sewer, phone, mobile phone, heating, waste disposal;
(11) financial - loan, credit, investment - record;
(12) insurance document;
(13) medical record;
(14) military record;
(15) internet, cable or satellite record;
(16) vehicle record (i.e. DVM Registration, Insurance Bill w/address)
