Please print out Registration From and Medical Release Form, to expedite registration. You will also need Birth Certificate and three proofs of residence Remember you must live in the following Boundaries:
Hiatus Road and Sunrise Blvd east to 441, south to Broward Blvd, east to SW 24 Avenue, south to New River, east to I95, south State Road 84, west to 30th avenue, south to Griffin Rd, west to 441, south to Stirling Rd., west to Hiatus Road, north to Sunrise Blvd.
