SPLL: 2014 Registration Information





Registration 2014  

 

Registration is now closed for the 2014 season.

Registration for Spring 2015 will be held in January!

 

***All players are required to fill out a Medical Release form, Concussion Acknowledgement form, and Code of Conduct  prior to practices/games.  All new players will also need to show a copy of their birth certificate as well as three proofs of residency.  Please get all forms in ASAP***  You may email them to registrar@southportagell.com,  fax them to (269)492-0675, or send to our PO BOX 523 Portage, MI 49081.

 

As of June 2013 All COACHES AND VOLUNTEERS must also complete a brief online concussion training and turn in your certificate to the league upon completion. We thank you for your cooperation with this.

2014 Pricing

 

First Player in household: $85 + fundraiser participation
Second Player in household: $65 + fundraiser participation 
Additional Players in household:

$50 + fundraiser participation

Big League and Seniors:  $75

 

*SPLL has a no-refund policy 

*If you want to opt-out of the fundraiser, all prices above will require an additional fee of $30 per player, due at registration.

Hershey's Assortments 

We will be selling Hershey's candy bars as a fundraiser this year.

Each box will cost $52 with each bar selling for $1

Here are the details of each box:

You may chose to opt-out of the candy bar fundraiser by paying a one time fee of $30.

All players must either sell candy or opt-out.

IMPORTANT NOTICE TO ALL PARENTS

SPLL Concession Stand Policy

Because of the importance of concessions as a source of revenue for the league, the SPLL board of directors has adopted the following policy:

All SPLL Families are required to volunteer for concession stand duty. As a parent of an SPLL player, you are required to volunteer for a minimum of 2 shifts per player in the concession stand during the season.  Only parents/grandparents are allowed as workers and no one under the age of 18 is allowed in the concession stand. We thank you in advance for your cooperation with this.

 

IMPORTANT NOTICE TO ALL COACHES/VOLUNTEERS

Concussion Law

As of June, 2013 the sports concussion legislation requires all coaches, employees, volunteers, and other adults involved with a youth athletic activity to complete a concussion awareness on-line training program.  Here is the link to the training http://www.cdc.gov/concussion/HeadsUp/Training/index.html 

ALL COACHES/ASSISTANT COACHES/VOLUNTEERS will be required to complete this training before entering the field. If you have already taken this training for another sport we will accept that certificate. You may email your certificate of completion to Ann Marie Breese at president@southportagell.com or send into our PO Box 523 Portage, MI 49081  

All PARENTS/PLAYERS will also be required to fill out a Concussion Acknowledgement Form stating that you have received the information and are aware of the symptoms and signs of a concussion.  You may email this acknowledgement form to Heather Ryan at registrar@southportagell.com or fax to (269)492-0675 or you may also send into our PO Box 523 Portage, MI 49081
