This TEAM / MANAGER REGISTRATION form is the only form you will will need to complete to become a part of SCWBA in 2008 if you get it in on time....

TEAM MANAGER REGISTRATION

A completed team roster showing all player's names and ages, the division, the team's name,and all manager's contact information, has to be turned into the League Player Agent by no later than the second weekend of the season.

TWO Copies for each player are required. After they are signed by parent, the original stays with team and the secord copy has to be sent into the league player agent.
