PERMISSION AND RELEASE OF LIABILITY AND PARENTS/GUARDIANS INDEMNIFYING AGREEMENT

I hereby grant permission for my child to participate in the activities of the Sudbury Minor Baseball Association. I am aware of the risks involved in playing baseball. Therefore, I will not hold the Sudbury Minor Baseball Association, its organizers, officials, members, directors or any other person or persons connected with it, liable for any injuries occasioned to my child during or as a result of their participation in the activities of the Sudbury Minor Baseball Association or while traveling to or from such activities.
I hereby waive any rights against the Sudbury Minor Baseball Association, its organizers, officials, members, directors or any other person or persons connected with it, and release them for any costs, claims, and/or liabilities.
A child under the age of eighteen (18) years, agree not to make any claim or take any proceedings against any other person or corporation who might claim contribution or indemnity under the provisions of any statute or otherwise against the Sudbury Minor Baseball Association, its organizers, officials, members, directors or any other person or persons connected with it.
I agree at all times to indemnify and save harmless the Sudbury Minor Baseball Association, its organizers, officials, members, directors or any other person or persons connected with it, against and from any and all claims, demands, actions and claims for contribution or indemnity whether under any statute or otherwise which may be made or brought by me or other persons on behalf of the said child or in any way arising out of any injuries suffered by said child as a result of or arising out of engagement in the activities of the Sudbury Minor Baseball Association directly or indirectly including the costs of defending the said action or claim.
