The deadline to sign up for any 2009 Fall sports program is Fri. July 31, 2009. You can drop them at the rectory office or in the Sunday collection basket. Please make sure to clearly mark envelope SMAA.

Registrations received after July 31 will be assessed a $25 late fee.

You must submit an SMAA registration form with payment as well as a completed, validated CYO Pre-participation Physical Exam form and completed Emergency Medical Authorization form by that date. 

If you take advantage of the Mobile Medical Service on Thurs. May 28, 2009 we will hold onto the physical form until we receive the registration form and payment. 

Practices may begin Monday Aug. 3 provided the athlete has completed the entire registration process. No one will be allowed to practice until the registration is completed.
