SJAsports: Have your medical form updated!

It is imperative you have your child's medical release form sent to the school office.  This form was attached to the SJA Summer Newsletter.  It will also be sent home in the upcoming SJA School forms mailing.

The medical release must be a current indication by your physician stating your child has had a physical in the past year.  These updated forms must be on file in the office so that your child can participate in practices and games.
