TOURNAMENT APPLICATION FORM

Completed Application - Teams must complete the following seven items to have a completed application on file with Showdown Tournaments. Further, teams must submit their completed applications in Microsoft Word to brian@showdowntournaments.com. Teams can fax (1-443-458-0188) or mail (to the address above) their signed and dated USA Hockey team rosters. Showdown Tournaments gives each completed application a date stamp once they receive all required six forms and full payment. We then use this date stamp to determine tournament acceptance and hotel placement. Teams please submit all six forms and full payment at the same time by the deadline. Partically completed applications and forms will not be considered completed and will not guarantee tournament or hotel placement for your team.

1. Application contact/signature page (pertinent for contacting teams and understanding caliber of play)

2.Tournament player roster (names for tournament program and must match USA Hockey team roster)

3. USA Hockey team roster (copy must be signed and dates by district register)

4. Hotel registration form (all non-local teams must stay at a host hotel. The form must include contact information, team credit card information, and each family’s name, number in room, and room type. All changes must be final one month prior to tournament. Showdown Tournaments will submit your form to the hotel approximately six weeks before the tournament). Teams do not book or contact hotels directly

5. Game Summary (this is pertinent for placing teams into the appropriate divisions. The form must include all scheduled games up to the tournament date and results for ames played to date.

6. Showdown Olympic registration form (submit original entry-you are not required to submit changes.)

7. Tournament Fee (make check payable in full to Showdown Tournaments and send to 1321 Swan Drive, Annapolis, MD 21409. Credit cards are not acceptable at this time).

1. Souvenir Preorder - The tournament souvenir preorder form provides all teams a great opportunity to purchase souvenirs at a discounted team rate prior to the tournament. This will also guarantee you get the souvenirs and sizes you want. Each year souvenirs sell out quickly!

2. Program Advertisment - the program advertisement (good luck/wishes) form provides every team and individual players the opportunity to purchase space in the tournament program. This will make a great keepsake for your hockey player and family for years to come.
