All players are required to complete the 2009 Player Registration form as well as a 2009 Medical Release form.
New players should submit a copy of their birth certificate.
Forms & payments can be dropped off anytime at the Shelburne Recreation office. Thanks!
Volunteers should have a copy of their drivers license when submitting a registration form.
Player/Team notification shall take place the last week of March 2009. Stay tuned!
