SHS:  Sports 2008-09
  • Participation Requirements for sports: Current Health form on file in the Activities Office (must be for the duration of the sport)
  • Forms MUST be submitted to the Activities Office prior to try-out date in order to try-out Start Date! No forms may be directly submitted to coaches.
  • Minimum GPA of a 2.0 the previous semester & pass 5 classes
  • Activity Fee – $160  Family Cap - $330  cash or check to: SHS  (no credit/debit cards) 
 
When signing up to participate in an activity please have the following: 
 
  • Health Exam form signed by doctor, student and parent/guardian.
    Health exams are good for one year and must be current through the season of the sport.

REGISTRATION:

*ALL FOOTBALL PLAYERS MUST HAVE A CURRENT PHYSICAL COMPLETED BEFORE REGISTRATION DAY (IT MUST NOT EXPIRE DURING THE FOOTBALL SEASON) 

*PARENTS MUST SIGN ALL ACTIVITY FORMS BEFORE GEAR CAN BE ISSUED AND ATHLETES CAN OFFICIALLY PRACTICE

*YOU MUST PAY THE $160.00 ASD ACTIVITY FEE (2008) ~ WAIVER FORMS ARE AVAILABLE FROM THE SHS OFFICE OR ASD WEBSITE

*ALL FOOTBALL PLAYERS MUST HAVE 10 PRACTICES BEFORE THEY ARE ELIGIBLE TO PLAY IN A GAME 



SPECIAL NOTES 
Remember - Grades will be checked for all student-athletes who want to participate in the Cougar Football Program.  Be sure to complete all your homework and turn it in.  Keep up to date on school projects and class requirements.  Turn off the TV and spend time studying for your tests.  Attend study hall and ask for help throughout the school year.  Make good choices…   Parents, teachers, coaches, and teammates are willing to help you be successful on and off the football field ~ but YOU have to do YOUR part!



COUGAR FOOTBALL TEAM RULES:

1. Be at all events on time with all of your equipment.

2. Always inform a coach about absences, no unexcused absences.

3. Demonstrate good sportsmanship at all times.

4. Be respectful of yourself, take pride in what you do, and how you behave at all times.

5. Be respectful toward others, including peers, adults and authority figures.

6. Be respectful toward your school AND the schools of others ~ honor and regard school rules and property at all times.

7. Follow all ASAA and ASD rules of conduct.      

8. Make good choices in everything you do.



CONSEQUENCES:


1. Any violation of the above rules as deemed by the coaches wll result in a steep penalty.

For example: an unexcused absence from practice will result in suspension for 1/2 of the next game. The second absence will result in 1 game suspension.

Coaches expect you to be responsible and understand that as a member of this team you must uphold all rules and be a positive model as a Service High School student athlete.
