YOU MUST FILE AN APPLICATION Click on the "Links" icon located on the left side of the Home Page of this web site. Then... Click on the 1st "Link"... 2009 Exceptional Srs. Application This is a link that will allow you to type out your Exceptional Seniors application and e-mail it directly to the Exceptional Seniors Coordinator. Simply... TYPE your application right on the web site and hit... SUBMIT Once your application has been received by the Ex. Srs. Coordinator your name will be posted below and you will be allowed to tryout. You are no longer allowed to send in your application by U.S. Mail IMPORTANT: Once your application is submitted the Exceptioanl Seniors Coordinator will know by the date posted on your e-mail if you met the deadline. If the date on your e-mail is not November 6th or earlier your application will not be accepted and you will not be allowed to tryout. If you show up at the first tryout you will be asked to leave ! Wednesday November 11th at 3pm @ The Patchogue/Medford Youth Soccer Complex on Sipp Ave. in Medford. * If you make the first "cut" your name will appear below and you must attend the second tryout session... Friday November 13th. 2nd tryout... Friday November 13th at 3pm @ The Patchogue/Medford Youth Soccer Complex on Sipp Ave. in Medford. * If you make the second "cut" you will be placed on one of the Suffolk teams. Your name will appear below and you MUST attend the practice sessions. COMMITTMENT FORM Players are required to sign a COMMITTMENT form before they leave the second tryout date. This committment form lets the coaches know that you will participate in the Exceptional Seniors games. If you sign the form and do not play in the games your name will not be mentioned at the SCSCA awards banquet in December. Go to: "Forms & Applications" Print out a Committment Form. Fill the form out & Sign the form. Bring the form with you. BOYS PRACTICE DATES 1st practice... Tuesday November 17th at 3pm @ The Patchogue/Medford Youth Soccer Complex on Sipp Ave. in Medford. Patchogue/Medford Youth Soccer Complex Sipp Ave. Medford, New York 11763 DIRECTIONS: Take the Long Island Expressway to Rt. 112. Go south on Rt. 112 toward North Patchogue. At the CVS Drug Store (which will be on your left) make a left turn onto Woodside Ave. Travel east on Woodside Ave. about 1 1/2 miles. The sign for the P/M Youth Soccer Complex will be on your left. Make a left onto Sipp Ave. The entrance is a couple hundred feet on your right.
