Saturday July 11th 10am - 1pm San Carlos Rec Center
FEES$250.00 tackle football and cheer
$125.00 flag football and cheer
Participant must be present at registration with Parent or Legal Guardian
Documents required at registration:
-Completed physical (completed on SDPW form and after 01/01/2009)
-Full year report card from 2008-2009 school year (min. 2.00 GPA)
-Original certified birth certificate and two copies-No Hospital Certificates accepted
-Cash, Money Order or Personal Checks will be accepted only
