Steele Canyon Youth Football and Cheer: Eligibility Requirements

Eligibility Requirements

You will need the following items to register:
1. Payment: Check, Money Order, or Cash (make checks/money orders out to SCYFB)

2. Proof of Residency: 2 sources ex: utility bill, cable or phone.

3. Proof of Age/Birth: ORIGINAL Birth Certificate (PLUS 3 copies) -
    ** Cannot be a Hospital Certificate!

4. Most Recent Report Card: 2.0+ GPA required (PLUS 1 copy) -

    ** Final report card will be required at the end of school year (PLUS 3 copies)

5. Picture: 1x1 head shot that will be kept by the Association .

6. Proof of Medical Insurance: Current documentation, card etc. (PLUS 1 copy) -   

    ** Military personnel only need to show their ID cards as proof of Tricare/Military Insurance (PLUS 1 copy).


7. Physical: Childs physical must be completely filled out and signed by a licensed practicing physician.

    ** Physicals signed by a chiropractor WILL NOT be accepted. All physical will be verified.

8. Final End of Year Report Card: 2.0+ GPA required (PLUS 1 copy)




Waivers:
All participants who live in one chartered area and request to play in another chartered area, must register in the area that he/she lives. The home Association will then forward the request for waiver to the Conference Executive Board for consideration. Participants who relocate during the season will stay with their original association and will be property players for the following seasons..
