Steele Canyon Youth Football and Cheer: Registration Information

1. Payment: Credit Card, Check, Money Order, or Cash (make checks/money orders out to SCYFB&C).

2. Proof of Residency: 2 Current Utility Bills (2 sources, 2 copies each)

3. Proof of Age/Birth: ORIGINAL Birth Certificate (PLUS 3 copies) -
                ** Cannot be a Hospital Certificate!

4. Picture: 1x1 head shot that will be kept by the Association .

5. Proof of Medical Insurance: Current documentation, card etc. (PLUS 1 copy) -
** Military personnel only need to show their ID cards as proof of Tricare/Military Insurance (PLUS 1 copy). 
 
6. Physical: Physical must be completely filled out, signed, and stamped by a licensed practicing MD physician. Physicals are due no later than JUNE 30th. (PLUS 2 copies)
**PHYSICAL FORM IS AVAILABLE ON THE HANDOUT PAGE FOR DOWNLOADING AND PRINTING. THIS FORM MUST BE USED FOR REGISTRATION!

** Physicals signed by a chiropractor WILL NOT be accepted. All physicals will be verified. If physicals are not received by June 30th, 2009, or it is discovered that the physical was completed by someone other than a physcian, the participant will be considered a "NO SHOW" and removed from the team or cheer sqaud, and placed on a waiting list.

7. Final End of Year Report Card: 2.0+ GPA required (PLUS 1 copy) . Final June report cards are due no later than JUNE 30th, 2009. This must be the final report card of the 2008-2009 school year, or the latest report card of the school. There will be no exceptions for any reason. Please obtain report cards before schools close for the summer. If the participant's report card is not received by JUNE 30th, 2009, the participant will be considered a "NO SHOW" and removed from the team/squad and placed on a waiting list.

Please bring all documents listed above, only participants who provide all documents on the day of registration will be placed on team/squads. Those with missing documents will be be placed on a waiting list. Sorry, there are no exceptions!
