This form is to be completed and turned in with the league fee for entry into the RRPD sport leagues.

Adult League Entry Form - 2008

This form must be completed by player and coach. The $25.00 roster addition fee must be paid at the Hughes Center. If the player is under 18 an underage waiver must be submitted with the roster addition form.

This form must be completed and turned in during the registration period along with the entry form and fee.

This contract and a deposit are required to book the Hughes Center. Call 479-968-1272 for available dates. The Hughes Center diagram will show room measurements and layout of the building.

Hughes Center Contract

Monthly Report - January 2008

Submit this contract with the rental fee to the Hughes Center office to reserve a city pavilion. Current fees are $35 for half days (8:00am to 3:00pm or 3:00pm to 10:00pm) or $50 for all day (8:00am to 10:00).

Pavilion Contract

The department begins booking pool parties on the first working day of May. Complete this form and pay the rental fee to book a date at M.J. Hickey Pool.

Pool Party Contract (4-29-08)

Submit this form with the rental fee at the Hughes Center to book a field for practice. Current practice fee is $10.00 for a 90 minute practice.

Practice Field Reservation Form

The smoker grill may be rented from the Recreation and Parks Department for $50. Non-profit organizations may reserve the grill at no cost to benefit their organization.

This contract is used to reserve facilities for a tournament. Contact the Hughes Center at 479-968-1272 for available dates.

Tournament Contract

This form and a $15 membership fee are required to become a member of the weightroom in the Hughes Center.
