This form is to be completed by each employee and volunteer who will have direct contact with ROSL youth participants.

(i.e. Managers, Assistants Coaches, Scorekeepers, Umpires,Team Parent, Board of Director, Base Coaches, etc.)

After completing this form please submit to the Manager. The Manager will then submit to the League Vice President and pass along to the Board of Director for a background check.
