2009 Coach's Application

To register your football player or cheerleader download and fill out the forms listed below. Bring three (3) checks and completed forms along with a (1) copy of your childs birth certificate (2) medical insurance card and (3) most recent report card to Ripon Chiefs Registration on March 28th for Returning Players/Cheerleaders and on March 30th for New Participants. See the link for Physicals on the home page for instruction on the medical form.

Registration Fees:

Football

First Player = $175
Second Player = $100

Cheerleading

First = $160
Second = $100

Volunteer Deposit = $200
Parent Volunteer time for one participant is 6 hours and for two or more participants it is 8 hours.

Equipment Deposit = $200 per participant
Deposit checks must be written for each participant.

Checks for Volunteer and Equipment Deposits will not be cashed and will be returned at the end of the season when volunteer hours are complete and all equipment has been returned. Registration fees can be combined on one check.

Example for signing up two football players:
Total of three checks written:
    One check for $275
    Two checks for $200
