2009 INDIVIDUAL PLAYER REGISTRATION FORM
To register by mail, you may download our Registration Forms located in our Download Forms link on the left side navigation bar or at the bottom of this page
Print the registration form and sign the form at the bottom right side of the page (medical and liability release). This MUST be signed by a parent, legal guardian, or player age 18. Players who are age 18 MUST sign their own registration form. The wet ink signature should be in a color. TWO copies of this form should be turned in to your League/Club Registrar with the appropriate fees. You may make a copy of the signed form and turn in the original and one copy OR you may turn in two signed copies.
This form must be filled out entirely (don't forget physician's name and phone number).

REGISTRATION FEES:
First Player: $125.00
Second Player: $100.0
Third Player and Beyond: $90.00
A late fee of $10.00 per player will be required beginning June 16, 2009.
The last day to register is June 30, 2009.

2009 GENERAL REGISTRATION INFORMATION
1. U6 PLAYER REQUESTS: RVYSC will make every reasonable effort to try to honor requests for coaches and friends as they put U6 teams together. However, sometimes this is not possible and RVYSC makes no guarantees that special requests will be honored.
2. U8 PLAYER REQUESTS: RVYSC will attempt to honor requests to put friends together on the same team with the following conditions:a. Only one “friend request” per player will be honored.b. Both children must request each other on their separate registration forms.c. Requests by Coaches will NOT be honored.d. There are no guarantees that special requests will be honored.
3. U10 and OLDER PLAYER REQUESTS: RVYSC will make NO attempt to honor requests for friends to play together or requests for a specific coach in the U10 and older age groups. All teams in the U10 and older age groups are formed with the intent of building teams of equal strength, experience and skill level.
4. COACHING REQUESTS: A Coach and Assistant Coach may request to work together and have their children on their team. If the coach’s request is in the U8 age group no additional “friend requests” will be honored for either of the coach’s children.
5. REFUNDS: RVYSC will provide a 100% refund for any requests submitted between April 1, 2009 and April, 30, 2009. RVYSC will provide a 50% registration fee refund for all requests submitted between May 1, 2009 and June 30, 2009 for medical reasons or for players moving out of the area only. No refunds will be provided after June 30, 2009.
6. DIVISION 3 TRYOUTS: Tryouts for Division 3 (“B”) teams will be held during the month of May. ALL registration forms (including player picture) and fees MUST be provided to RVYSC prior to attending tryouts. Any player that does not make a Division 3 team will automatically be place on a Division IV (Recreational) team. Standard refund policy will apply.
IMPORTANT PLAYER REGISTRATION DETAILS
1. All recreational players in the U14, U16 or U19 age groups and ALL Division III players must include a photo (1 inch x 1.5 inches), full face (size of a nickel), NO sunglasses or hats. NO laminated pictures please.
2. Players that did not play with RVYSC last year (2008) must provide proof of age for Registration. A copy of one of the following affidavits are acceptable: Birth Certificate, Birth Registration issued by an appropriate Government Agency, Board of Health Records, Passport, Alien Registration Card issued by the United States Government, a certificate issued by the Immigration and Naturalization Service attesting to age, or a certificationg of an American citizen born abroad issued by the appropriate Government Agency, a current Drivers License, or a current Identification Card issued by the Department of Motor Vehicles. The following affidavits will NOT be accepted as proof of age: Hospital, baptism or religious certificates, Certification of Birth for school registration/employment only.
3. Players must be born between August 1990 through July 2005.
4. Registered players will be rostered on teams by age. If a player seeks to participate with an older age group, parents must complete and submit a Play Up Request Form during registration. This form is available in our Download Forms link located on the left side navigation bar. Completion of this form does not guarantee a play-up request.
5. Players may register with any club or league. Each club in Santa Rosa uses the boundaries to define where its teams will practice and play their games.
6. No youth is to participate in team practices or competitions without being registered with their club as required by CYSA.
7. A player shall be deemed "registered" effective the date the completed paperwork and fees (or scholarship application) are received by the Club Registrar or designee.
8. Players attending Division 1 tryouts prior to league-wide registration must either complete registration before tryouts or be CYSA registered the previous playing season.
10. A player is bound to a team for the seasonal year once officially placed (roster signed by a league official), unless an official transfer or released is obtained. No changes can be made to a team's roster without approval of the Club and League Registrar. Player add, release, or transfer forms must be filed and any fees paid. A player transfer request requires the signature of both coaches, the player and a parent. Only three transfers onto a team are allowed each season. A player release must be signed by the player, a parent and the coach.
