The following is the form that youth umpires will need to print out and bring to a Rookie game. Rookie managers should try to keep a few blanks as well. The rookie umpire will fill it out with the manager and then send it in as instructed on the sheet for payment.

Please fill out and submit the background information form (w/copy of Driver License). This is required each year on a national basis for any person interested in coaching or managing in little league.
