To Mail-In a Registration, please click on (Click Here) just above. The Registration Form will open, print it, fill it out, and mail it in. You will need to have Adobe Reader installed for this process. (Allow 2 weeks for processing).

The Ralston Soccer Club Medical Release Form
Players will need to have a current Medical Release Form for each season. Parents, this form gives your child's coach permission to seek medical care for your child if they are injured at a practice or a game that you are unable to attend. Please be sure to print a copy, complete, and give it to your coach when the season begins. At last check, notarization is not required in Nebraska but if your child's team will be participating in tournaments please check each tournament's rules to be sure notarization is not required.
Please give the medical release form directly to your child's coach!

Additional Information:
In house/micro games are played on Saturday mornings with rainout makeups played on Wednesday evenings. All of the In house/micro games are played at the Ralston Fields at 81st and Serum Ave. ENSA U9-U19 play on various days and various field locations. Go to www.ensa.us to see what day your child might play based on age. All teams are coached by volunteers and practice schedules are determined solely by each volunteer.
