The following forms are required to register your child for football/cheer. Please note the due dates for the forms.

• 2009 Registration Form - Due at registration
• 2009 Parental Addendum - Due at registration
  
• 2009 Player Contract - Due at registration
• Your child's ORIGINAL birth certificate (for first-timers only) - Due at registration
• Registration Fee: - Due at registration (see registration flier)
  
  • Family Discounts available: First Child Full Price, $10.00 off each additional Child.

• 2009 Medical Form - Due July 15, 2009 (tentative date) PLEASE NOTE THAT YOUR CHILD(REN) WILL NOT BE ALLOWED TO PARTICIPATE UNTIL THE MEDICAL FORM HAS BEEN SUBMITTED, NO EXCEPTIONS
  
• A current wallet-sized photo of your child - Due July 15, 2009 (tenative date)
  
• TWO (2) complete copies of your child's June 2008 report card (front and back) - Due July 15, 2009 (tentative date)
  

***If you have an outstanding prior balance for any reason (bounced check fees, 2008 registration fees, lost equipment, etc., YOU WILL NOT BE ALLOWED TO REGISTER YOUR CHILD UNTIL ALL PAST DUE BALANCES HAVE BEEN SATISFIED. These past due balances are payable in CASH OR MONEY ORDER ONLY.
