| Permission Forms
In order to participate in athletics the state requires students to submit complete physical forms and permission (health/history) slips. The required forms may be accessed below. Form A is the health/history form. It should be completed by a parent/guardian. Form B is the physical form. It must be completed by a physician. All forms must be submitted to the school nurse complete. Incomplete forms will no longer be accepted.
The Pascack Valley Regional District requires all athletes (and parents/guardians) to sign a sportsmanship contract and a substance abuse and tobacco policy form. Athletes are not permitted to participate without signing and returning the contracts to the director of athletics. (This is the third form listed below.)
The NJSIAA has passed legislation requiring all student/athletes and their parents/guardians to sign a "steroid testing permission slip." The permission slip is listed below; failure to sign the form will render the student athlete to be ineligible for competition. For the NJSIAA list of banned substances and other information, go to;

Physical Form - A
