Online Registration
Online USAV Registration.  

If you are a returning USAV member or new to USAV, please click on the appropriate link below.  If you are renewing your USAV membership and you don't remember your username and password or the email account you used to register last year, please call John or Amy at the Puget Sound Regional (PSR) office, 425-673-4103, for assistance.  You may also email the PSR office at usavpugetsound.office@verizon.net. 

          New USAV Member Registration

          Returning USAV Member Registration

You are registering at webpoint to become USAV members, or in the case of returning players, to renew your USAV membership.  Do not pay or be concerned about the USAV registration fee at this time.  Your USAV registration fee (not to be confused with the $15 Power Surge tryout fee) is included in your club dues, the startup/initial fee in particular.  We will send the USAV registration fee into the Region after we form our teams and collect the startup/initial fee from each player.  Print, complete, and sign where appropriate the following USAV forms and bring to the tryout.  
 
     1.  Waiver and Release of Liability Form - Parents must sign by entering their driver's license number.  Please bring this form to the tryout.
     2.  Medical History Form - Complete and bring to the tryout
     3.  Membership Application Confirmation (MAC)- Please keep the MAC for your records.  This form has your email account and password, which you will need to access your USAV account to print your membership card and to renew your membership next year. 
     4.  Code of Conduct - Please read and keep for your records.

If you are selected to play on one of our teams, only then will you need to complete and sign the following forms.

     Player Contract To Play

Everyone attending our tryouts are required to complete our Power Surge Tryout/Registration form before participating in the tryout.

Coaches:  Coaches must also renew their USAV membership each year or register as a new USAV member.  Please complete, sign, and bring the Background Screening form (required every two years) if it applies to you, along with your Waiver & Release of Liability Form.  Read and keep the Code of Conduct form for your records.
